Carotid Stent Check ListAfter Roubin and Iyer

• 7-8F 90 cm Sheath or 8-10F Guide

• Choose a Stent

  • Self-Expanding (Wallstent, SMART stent etc)

    • Most commonly 10 x 2 mm
  • Balloon-Expandable (Palmaz)

• Access the Carotid and wire the lesion

  • e.g., 125 cm 5F HN2 through sheath or guide with Glide wire (0.038”)
  • Extra Stiff Amplatz high in External Carotid if necessary
  • 0.014 or 0.018 stiff support wire in ICA

• Predilate (e.g., 4 x 40 mm coronary balloon 10-12 ATM)

• Post Dilate (e.g., 5-5.5 balloon)

• If TIMI 3 flow in External Carotid, leave it alone

• Pacer/Inotrope ready for heart block and hypotension
